Please note: Available for new patients only. All three items need to be scheduled and completed in the same day and your initial cleaning does not include any further cleanings if found that you may require them in the case of periodontal disease.

Yes, Payment is expected at the time services are rendered.
While we are not in network, we accept most PPO insurance plans. we are contracted with Delta Dental as a Premiere provider. Please contact Us for more details. Our accounting department will prepare all of the necessary forms for this important benefit. However, we remind you that your policy is an agreement between you and your insurance company, not between your insurance company and our office. We can make no guarantee of any estimated coverage, but we’ll work hard to ensure that you receive your maximum benefits.

Please call our office at least 48 hours in advance of your appointment. We'll work with you to help you find another date that fits your schedule.
So that we may be mindful of our patients’ valuable time, we ask that you give our office 48 hours advanced notice if you are unable to keep your appointment. Appointment cancellations are not accepted out of business hours. In that was we may extend convenient appointments to all our patients. Late cancellations are subject to a service charge.
Yes, Dr. Levine sees children of all ages. Starting as little as 1 years old.
Dr. Levine covers all procedures including Happy Visits, exams, cleanings, fluoride applications, fillings if needed, pediatric extractions if needed, and evaluations for orthodontic referral if needed.
